Request a Certificate of Insurance

This page is for lenders, settlement attorneys, realtors, and homeowners seeking proof of Master Policy insurance coverage either to close a new loan or refinance an existing loan.

To request a certificate of insurance, please note that we will need to have the following information available in order to fulfill your request:

1) Name of the condominium, cooperative, or

        homeowners association.
2)     Full name(s) of the borrower(s)/purchaser(s).
3)     A complete unit or street address of the property being financed, including the

        city, state, and zip code.
4)     Complete name of the lender.
5)     Lender's loss payee information/mortgagee clause.
6)     Lender's complete address, including city, state, and zip code.
7)     Lender's loss payee position (1^st Mortgagee, 2^nd Mortgagee, etc.)
8)     The borrower's loan number (and Escrow Number if Closing Agent is

initiating the order).
9) The preferred method of delivery of your certificate (e-mail, fax number,

or mailing address). E-mail is the fastest method. (Receiving certificates via

e-mail requires that you have Adobe Acrobat Reader downloaded in order to

view the full certificate.) We respect your privacy. Your e-mail address and

contact information will not be shared.
