Request a Certificate of Insurance
This page is for lenders, settlement attorneys, realtors, and homeowners seeking proof of Master Policy insurance coverage either to close a new loan or refinance an existing loan.
To request a certificate of insurance, please note that we will need to have the following information available in order to fulfill your request:
1) Name of the condominium, cooperative, or
homeowners association. 2) Full name(s) of the borrower(s)/purchaser(s). 3) A complete unit or street address of the property being financed, including the
city, state, and zip code. 4) Complete name of the lender. 5) Lender's loss payee information/mortgagee clause. 6) Lender's complete address, including city, state, and zip code. 7) Lender's loss payee position (1st Mortgagee, 2nd Mortgagee, etc.) 8) The borrower's loan number (and Escrow Number if Closing Agent is
initiating the order). 9) The preferred method of delivery of your certificate (e-mail, fax number,
or mailing address). E-mail is the fastest method. (Receiving certificates via
e-mail requires that you have Adobe Acrobat Reader downloaded in order to
view the full certificate.) We respect your privacy. Your e-mail address and
contact information will not be shared.
